In order to ensure timely processing of your application, please submit the following:
- Driver’s license or State ID
- Copy of your Social Security Card
- Proof of income (e.g. 2 recent pay stubs, S.S.I/Disability, retirement, etc.)
- 2 years of tax returns and 3 months of bank statements (self-employed only)
